The lapse is when Napenas found it worthwhile to brag about the mission being accomplished than to make sure that the Oplan be implemented to the letter. Up to 3 PM all his lines to SAf 84th and SAF55th was clear. How can he possibly explain why he suddenly forgot that he had 348 other fully armed SAF commandos at his disposal. That is the big blunder. If the president had talked to his direct subordinates, Espina and Catapang, this would not have happened.
